Summary Plan Description

We have attached/linked below the Summary Plan Description (SPD). This document describes the health and welfare benefits available to you (and if applicable, your family) as an eligible participant in the Plan as of the 2024-2025 plan year. This SPD outlines your rights and responsibilities under the Plan and reflects your benefits as they currently are. Additionally, the provisions in this SPD are intended to meet all federal statutory and regulatory requirements currently applicable to the Plan.

We recommend that you keep this SPD with the various documents provided to you upon enrollment in each benefit plan (insurance contracts, evidence of coverages and any other official plan documents). Additionally, we suggest that you also share the SPD with any family members you have elected to cover under the Plan.

You also have the right to request (at no charge) a paper version of the SPD and Plan document. If you have any questions, have problems accessing the SPD, or require a paper copy please contact Human Resources:
