Hospital Insurance

This product serves as a companion to your health insurance to take the financial sting out of a hospital stay. Lincoln’s Hospital plan provides funds that can complement your medical plan copays and deductibles. The plan includes a $1,000 or $2,000 admission benefit plus pays $150 or $200 per day you are confined to the hospital. The plan also includes a $50 Wellness benefit for completing a covered health assessment.

The amounts shown below are BI-WEEKLY amounts (26 payments/deductions per year). You may elect insurance for you only, or for your family. You have a choice of plan options. Premiums will be automatically deducted from your paychecks as authorized by you during the enrollment process.

HOSPITAL HIGH COVERAGE TIER PREMIUM AMOUNT:
Employee/Member $15.13 ($1.08 per day)
Employee/Member + Spouse $29.58 ($2.11 per day)
Employee/Member + Child(ren) $23.37 ($1.66 per day)
Employee/Member + Family $41.57 ($2.96 per day)

HOSPITAL LOW COVERAGE TIER PREMIUM AMOUNT:
Employee/Member $9.63 ($0.69 per day)
Employee/Member + Spouse $18.38 ($1.31 per day)
Employee/Member + Child(ren) $14.82 ($1.06 per day)
Employee/Member + Family $23.58 ($1.68 per day)
