Accident Insurance

The accident plan covers unexpected expenses that result from on or off-the-job accidents -sports-related or household. It provides a cash benefit to cover things your health insurance plan doesn’t such as deductibles, copays, transportation and lodging, and everyday bills. Also, your benefits come directly to you without restrictions on how you use them. This plan includes a wellness benefit of $50 per person (Employee and Dependents covered on the plan) annually for routine physicals, immunizations, and health screening tests.

Employee/Member $3.30 ($0.23 per day)
Employee/Member + Spouse $5.51 ($0.39 per day)
Employee/Member + Child(ren) $6.98 ($0.50 per day)
Employee/Member + Family $9.77 ($0.70 per day)
